What can cause shoulder and upper arm pain?

Shoulder symptoms can involve the joint, muscles, tendons or ligaments, and can sometimes be referred from the neck. Possible explanations include rotator cuff problems, stiffness associated with a frozen shoulder, arthritis or an injury. These possibilities need assessment; pain with one movement cannot confirm a tear or other diagnosis.

When should shoulder pain be assessed urgently?

Call 000 for shoulder pain with chest pain or shortness of breath. After an injury, an unusual joint shape, sudden swelling, intense pain or loss of feeling or movement needs urgent medical attention. Fever with a hot, red or swollen shoulder also needs prompt assessment. Do not try to put a suspected dislocated shoulder back into place yourself.

Describe the movement that is difficult

Reaching a shelf, dressing, carrying bags and doing overhead work place different demands on the arm. Before booking from Reservoir, write down which movement is limited and whether symptoms began suddenly or gradually. Tell the clinician about falls, sporting contact or a previous dislocation.

Bring useful training and work details

Note recent changes in gym load, throwing, racquet sport, repetitive work or time using tools. Bring the weights, repetitions or task duration you were managing before the problem. This helps you discuss a realistic starting point and review target.

What to ask at the assessment

  • What needs to be examined before choosing treatment?
  • Do these symptoms need medical review or further investigation?
  • Which tasks can we use to measure change?
  • What should I do if strength or sensation changes?

Plan around your own priorities

Tell the practitioner whether your priority is comfortable sleep, returning to your job or a particular sport. Ask for a written home plan and a review point. If equipment or time is limited, raise that before leaving.

Understand the terms in your report

A scan label needs to be discussed alongside your history and examination. Ask what the finding means for your function and whether it changes the plan. Avoid choosing a treatment solely from a report description.
